This training will prepare participants to facilitate best practice transition planning to support youth and families utilizing System of Care Principles.
* Describe the Phases of the Wraparound Planning Process
* Describe the Phase of Transitions
* Identify Best Practices related to Transition Planning
* Practice utilization of Best Practices
Participants will be provided with resources that include the following: Service Profile for youth in transition, Best Practice Checklist, Best Practice Instructions
Participants shall have had previous training in System of Care, Child and Family Teams, and Facilitation of Child and Family Teams
