Accelerating Positive Outcomes through Easy to Understand (and Successful) Practices

Positive outcomes for residents of elder care communities don't just happen by chance. High quality of care and high quality of life are a direct result of good staff paired with effective systems of care and support. This session will highlight four key practices that, when used together, create a foundational system for achieving those high quality outcomes. Individually, the practices of consistent assignment, shift huddles, involving CNAs in care planning, and QI closest to the resident hold their own merit, and when used together as an interrelated system of care, each single practice is strengthened and mutually reinforced, resulting in even better outcomes.
Join us to hear how 49 nursing homes from across the country, including three Oregon homes, joined a national Pioneer Network initiative to take on the work of making significant changes in these practices. learn what these homes did to accelerate culture change and quality improvement when they provided the infrastructure for staff to know residents well and communicate routinely about residents needs as part of day-to-day work.
