Whether you've been on the culture change journey for a long time or haven't yet started, this program is for you!  Join us for an afternoon of examining how to initiate or re-examine the comprehensive organizational systems necessary to support high quality, individualized care.

In most organizations, we are trained to ask, "What's wrong?" and "How can we fix it?" This is a typical and yet demoralizing process.  Instead, consider asking two very different questions: "What's possible here?" and "Who cares?"

In this session, you won't just listen to theories. Instead, you will have the opportunity to hear how three Oregon providers took on the hard work to make significant changes in three key organizational practices:
- Dedicated staff assignments
- "Huddles" to regularly communicate and problem solve with co-workers
- Including direct care workers in care planning

Culture change experts will tell you that as nursing homes and assisted living communities change their systems to increasingly engage the entire team and support residents in their customary routines, positive outcomes are promoted and negative outcomes are reduced. In this session, you will have the opportunity to hear what worked and what didn't work for these Oregon providers, what it took to engage staff in the process, and the actual outcomes that they are experiencing.

This session provides a "toolkit" good for novices as well as early adopters who will want to review their practices for fidelity and refinement.  

Who should attend:
All staff in nursing homes, assisted living and residential care communities who are involved in any way supporting residents to live their best lives. This includes but is not limited to Administrators, CNAs and personal care aides, nurses and Directors of Nursing, Social Services Directors, Activity Directors, Dietary Managers and other department managers.

Presenters
Lynda Crandall, RN, GNP with team members from Rose Villa, Friendsview Retirement Community and Cornerstone Care Option, who all participated as incubator nursing homes in the Pioneer Network National Learning Collaborative over the past 15 months.

Learning Goals
- Analyze how well current systems foster inclusion of everyone.
- Define dedicated assignment, huddles and inclusion of direct care workers in care conference, and examine a process for achieving these practices
- Discuss the impact of shared leadership as a key ingredient to successful culture change
- Identify ways to promote active engagement and critical thinking for effective change on a day-to-day basis.
