Train the Trainer: Best Practice Transitional Care Training
This training will prepare participants to facilitate best practice transition planning training and coaching to support commuity partners in utilizing System of Care Principles to plan for healthy and positive transitions.
* Train on the Phases of the Wraparound Planning Process
* Train on the Phase of Transitions
* Train on the Best Practices related to Transitions
* Coach on the implementation of the Best Practices on a practice level
Potential trainers shall have experience and knowledge associated with System of Care, Child and Family Teams, and Wraparound Planning
Participants will be provided with resources that include the following: Service Profile for youth in transition, Best Practice Checklist, and Best Practice Instructions
