Attention award winners!
If you have not emailed our President with your name and food choice please do so now! Her email address is firstname.lastname@example.org. Also, please do NOT include your name and menu choice on Brown Paper Tickets, as your meal is complimentary. Adding your name and food choice to Brown Paper Tickets will give us an inaccurate headcount.
Event details as follows:
This year's awards ceremony will be held Saturday, Oct. 12, 2013, at the Flames Eatery, Banquet Room, 88 S. 4th Street, San Jose, Ca. 95112. Phone number is 408-971-1960. Website: www.flameseatery.com.
Registration will begin promptly at 5:00pm, and will end at 5:25pm.
Seating is based on a first come first serve basis.
The event will begin promptly at 5:30pm and will end at 8:30pm.
Please bring your parking tickets to the event so that the Flames can validate your ticket.
Individual winners may bring two guests at a cost of $25.00 per person.
Organizations may send two representatives for free.
Cost: There are no refunds!
$50.00 per person.
$35.00 for seniors 65 years and older and students. Proof of ID for senior and student ticket purchases will be required during registration the day of the event.
Please be advised that the banquet room has an occupancy limit of 200, so please be sure you purchase you tickets now.
All tickets must be purchased in advance, as there will be no ticket sales at the door.
Ticket sales end promptly Tuesday, Oct. 1, 2013, at midnight.

Entree Choices Are:
The Flames Steak, garlic mashed potatoes, mixed vegetables.
Half Oven Roasted Chicken, garlic mashed potatoes, mixed vegetables.
Fresh Pacific Salmon, garlic mashed potatoes, mixed vegetables.
Vegetarian meal: Capellini de angelo (angel hair pasta, eggplant, roasted peppers, sun dried tomatoes, garlic, and light marinara sauce)
(Please note: Everyone will receive a garden fresh salad, dinner rolls, one entree, and a non alcoholic beverage of their choice. Coffee and cake will be served after dinner. Coffee is free after dinner but may be purchased at the No Host Bar, prior to, or during the dinner.)
