Takashi Hayama, an ordinary third-year high school student with his university exams up ahead of him, runs into a girl named Azusa Oribe. He falls in love with her at first sight, and takes her out on several dates. Eventually, she makes a strange confession.
"Everyone that I meet forgets me a few hours later. For some reason I don't understand, I disappear from their memories."
Takashi finds it impossible to believe her ridiculous story, but later, he suddenly realizes that his recollections of Azusa are fading. "Who did I meet yesterday?" "Who am I supposed to be going on a date with today?"
In a desperate attempt to prevent himself forgetting Azusa, Takashi begins to make detailed notes of their date plans and everything that happens when he is together with her. However...
