Frac Induced Failures and Investigations
Abstract:
This presentation reviews events and subsequent investigations related to tubular failures in horizontal multi-frac projects. Specific steps from initial information from the rig site or recovered (fished) pieces to material and fracture face examination are documented with photos for each step of the procedure. Participants will come away with an appreciation of how a failure can be turned into a learning opportunity and avoid similar mishaps in the future. A comprehensive data base allows us to not only show examples but characterize the most common causes of problems. Case studies picked from 40 years of failure investigations in the Permian, Eagle Ford, and Haynesville are reviewed for lessons learned.

Bio:
For most of his career, Bob has been in pivotal major project planning, scheduling, and coordinating assignments with major operators. His seventeen years of experience as a drilling and completions engineer with a major petroleum producer includes quality assurance expertise in the manufacture of equipment for high pressure and sour gas production, drilling and subsurface operations. Bob's current work encompasses 3rd party certification of well operations including plugging and abandonment designs, HPHT casing design, connections, and material selection for deep, high pressure, sour wells. Bob has provided certifications for over 400 domestic (BSEE Gulf of Mexico) and international well operations.
Bob earned his Bachelor's degree in Civil Engineering from South Dakota School of Mines
and Technology in 1975. He spent seventeen years with Exxon Company, USA, where he
led the Mobile Bay Completions Technology Quality Assurance Team. Bob left Exxon to join
Oil Technology Services as Vice President - Engineering. While with OTS, Bob managed the
drilling, completions, and production of technically complex wells. Bob co-founded VIKING
Engineering, L.C. in 2001.
Bob has authored several design manuals, SPE papers and technical articles for industry publications. He is a registered professional engineer in the State of Texas.
