Care Transitions: Continuing the Conversation
Care Transitions: Continuing the Conversation is a one-day conference for health care professionals, human services providers, and community leaders that focuses on improving patient transitions among care settings in order to prevent unnecessary hospitalizations. South King County is our primary focus; however, participants from nearby areas are also welcome.
On June 5 of this year, more than 160 individuals, representing 70 organizations, gathered at the same site to discuss key strategies and processes for successful care transitions. Workshops included Discharge Planning from Day One, Frequent Flyers: Developing Community-based Strategies to Serve Frequent ER Visitors, Home and Beyond: Long-term Services & Supports, Managing Depression, Anxiety and other Psychiatric Symptoms While Making Healthy Transitions, and Medication Challenges in Care Transitions: Issues Faced by Patients, Providers & Community Professionals.
Build upon those conversations by registering for and attending Care Transitions: Continuing the Conversation on October 2, 2014.
REGISTRATION: Must be completed online or by mail in advance. No registration is available on the day of the event.
MEALS: This event will include a buffet continental breakfast and buffet lunch. Vegetarian options will be available.
ACCOMMODATIONS: When you complete your online registration, you will be asked if you require an accommodation in order to to attend and participate. Every effort will be made to provide reasonable accommodations such as special seating, ASL or CART. Please try to place your request by September 11, 2014.
