Head and Neck Cancer Exam for the Dental Team

Head and neck cancers include malignancies of the mouth, throat, salivary glands, sinuses, nose, and larynx.  These cancers tend to metastasize to the lymph nodes within the neck.  Dental practitioners may be at the front line for detection and diagnosis. Head and neck pathology can be detected with a thorough intra-oral, extra-oral and radiographic examination. The proper technique to perform a comprehensive exam and take a good history will be presented.

This course will help the dental team identify the signs and symptoms of head and neck cancer and outline the risk factors for development. We have known for many years that the most common risk factor is tobacco use and the combination of alcohol and tobacco leads to an even greater risk. We now know that infection with the human papillomavirus (HPV) is a risk factor for cancer of the back of the throat, base of the tongue, and tonsils.
This course will discuss in detail the role of HPV in head and neck cancer and define what HPV is, how it is transmitted, diagnosed, and prevented.

The course will provide examples of clinical abnormalities that may be present in the soft tissues of the face, neck, oropharynx, and oral cavity and discuss when to biopsy and how to perform the procedure. Though potentially lethal, cancers of the head and neck are highly curable, particularly if detected early. There are many commercially available adjunctive techniques and devices for early detection of premalignant lesions and oral cancer and these will be presented in detail.

Head and neck cancer and the therapies used to treat it can sometimes lead to short- and long-term oral and dental problems. Most commonly oral mucositis, infections, radiation caries, hyposalivation and trismus may result. The dental management of these acute and late reactions will be discussed including hygiene considerations and prevention and management of caries. The pre- and post-therapy considerations for the dental team will be presented.

Dr. Jennifer Frustino is currently the Director of Oral Cancer Screening and Diagnostics in the Division of Oral Oncology and Maxillofacial Prosthetics at the Erie County Medical Center in Buffalo, NY.  She is a graduate of the University of Rochester where she earned a BA degree in Biology and Psychology and an MS in Natural Sciences at the University at Buffalo Roswell Park Division. She was awarded a DDS degree from the University at Buffalo, and a PhD in Cancer Pathology and Prevention from the University at Buffalo Roswell Park Division. Dr. Frustino did her postdoctoral training in Dental Oncology and Cancer Prevention and Population Sciences at the Roswell Park Cancer Institute. A few of her previous hospital affiliations include Roswell Park Cancer Institute, Brigham and Women's Hospital Division of Oral Medicine and Dentistry, Dana-Farber Cancer Institute Division of Oral Medicine and Dentistry and Harvard School of Dental Medicine Department of Oral Medicine, Infection and Immunity.
